Cosmetic Nose Surgery (Rhinoplasty)
Cosmetic nose surgery, also known as rhinoplasty, is a surgical procedure performed to correct breathing problems and aesthetic deformities of the nose. These issues may be congenital or may develop as a result of trauma or injury to the nose.
In some cases, individuals choose to undergo rhinoplasty purely for aesthetic reasons. For example, in East Asian countries such as South Korea, patients may request the creation of a nasal hump. In these cases, the cosmetic surgeon may use rib cartilage, pelvic bone, or synthetic implants to create a nasal bridge through septorhinoplasty.
In contrast, in countries such as Iran, patients commonly request removal of the nasal hump and the creation of a gentle nasal curve instead.
In recent years, cosmetic nose surgery has become increasingly popular, and many individuals seek rhinoplasty using the latest surgical techniques.
Over time, rhinoplasty methods have undergone significant changes, resulting in more satisfactory outcomes compared to the past. This article introduces the newest rhinoplasty techniques and discusses the advantages and disadvantages of each method.
Rhinoplasty, or cosmetic nose surgery, is one of the most popular surgical procedures worldwide, particularly in Iran, and many patients undergo this surgery on a daily basis. Depending on the type of nose and the surgeon’s diagnosis, the procedure may be performed using different techniques. During surgery, incisions are made inside the nose, allowing the surgeon to modify the nasal bones and cartilage and change the shape of the nose.
Rhinoplasty also includes revision (secondary) nose surgery, in which the surgeon focuses on repairing and reconstructing the shape of a nose that has previously undergone surgery.
Septoplasty, or surgery for a deviated nasal septum, refers to correction of deviation or curvature of the nasal septum. Negative consequences of a deviated septum include increased nasal discharge, nosebleeds, pain, and breathing difficulties. For this reason, patients with significant septal deviation often require surgical treatment to improve nasal function.

Types of Rhinoplasty Techniques
Open Rhinoplasty
In the open rhinoplasty technique, the nasal skin is completely separated from the underlying cartilage and bone structures, providing the surgeon with full and direct access to the internal anatomy of the nose. This allows the surgeon to perform more extensive and precise modifications to different nasal components.
Open rhinoplasty is considered a more accurate surgical method and is commonly used for patients who require significant changes to the shape of their nose. In this technique, placement of cartilage grafts is also easier, and all secondary (revision) rhinoplasty procedures are performed using the open approach.
In cosmetic nose surgery using the open method, complete elevation of the nasal skin enables the surgeon to clearly visualize, measure, and analyze the internal nasal anatomy. This level of access is not available in closed rhinoplasty.
Closed Rhinoplasty
In closed rhinoplasty, the surgeon makes only two incisions inside the nostrils, with no external cuts. For this reason, this method is generally suitable for minor corrections, such as improving the appearance of the nasal tip.
One of the main advantages of closed rhinoplasty is reduced swelling and a shorter recovery period. Compared to open rhinoplasty, postoperative swelling is significantly less, the nose heals faster, and the recovery process is generally easier and shorter. Additionally, skin trauma and visible scarring are minimal with this technique.
Every Face Deserves Beauty and Healthy Breathing
Dr. Farshid Achak provides specialized rhinoplasty (cosmetic nose surgery) services based on experience, precision, and up‑to‑date medical knowledge. If you are looking for a safe, professional, and facially balanced transformation, now is the right time to schedule a consultation.
Pre‑Operative Steps for Rhinoplasty
In‑Person Consultation
The first and most important step before undergoing cosmetic rhinoplasty is to determine whether you truly need nose surgery or not. The most accurate and reliable answer can only be obtained through a direct medical examination by the surgeon.
Information provided through online consultations or image‑based assessments cannot be considered fully accurate or dependable. An in‑person consultation allows for a precise evaluation of nasal structure, facial proportions, and breathing function.

3D Scanning and Rhinoplasty Result Simulation
For the first time, using the latest hardware and software technologies, candidates for cosmetic rhinoplasty undergo a three‑dimensional facial scan at the clinic. This process creates a high‑precision 3D model of the patient’s head and face.
In the next step, Dr. Farshid Achak, while carefully considering all scientific principles and strictly respecting breathing and health safety standards, applies the patient’s preferences and desired changes directly to the 3D facial model.
With this advanced approach, patients can view their post‑rhinoplasty appearance with exceptional accuracy on a highly realistic three‑dimensional model. This allows them to discuss, review, and refine the planned changes in close consultation with the surgeon before the procedure.
CT Scan and Sinus Endoscopy (When Required)
In specific cases and based on the surgeon’s diagnosis, a CT scan of the face and sinuses may be required for a more detailed evaluation. This is usually recommended when a severe deviation of the nasal septum is detected during clinical examination or when the patient has a history of chronic sinus conditions or disorders.
It should also be noted that if sinus endoscopy is necessary, all required advanced diagnostic equipment is available at the clinic. This diagnostic procedure is performed directly by Dr. Farshid Achak to ensure accurate assessment and proper treatment planning.

Blood Tests Before Rhinoplasty
To proceed with rhinoplasty surgery, patients are required to complete two stages of blood testing.
The first blood test is performed immediately after the consultation session and will be conducted at a designated laboratory.
The second blood test is required approximately one month after the consultation and about seven days before the scheduled surgery date. This test can be completed at any accredited medical laboratory.
Please send a clear image of the second blood test results via Telegram. If any abnormal findings are identified, the medical team will take the necessary steps and make the appropriate arrangements prior to surgery.
Finalizing Your Rhinoplasty Surgery Date
After completing the consultation process and reviewing the required medical tests, if no issues are identified, you may proceed to finalize your rhinoplasty surgery date.

Pre-Operative Instructions
Post-Operative Care
Essential Pre‑Operative Care Before Rhinoplasty
Anesthesia‑Related Instructions
Avoid consuming dairy products starting three days before surgery.
Trim nasal hair only with an electric trimmer within two days before surgery. Do not use chemical methods such as wax, depilatory creams, or tweezers.
Do not eat or drink for at least eight hours before surgery. You must be completely fasting for general anesthesia.
Remove all nail polish from both fingernails and toenails. If you have artificial or acrylic nails, they must be completely removed from hands and feet.
Hygiene Instructions
Take a shower on the morning of surgery.
Female patients must arrive at the hospital without facial makeup.
Male patients must shave their facial hair completely or trim it to zero length using a razor or clipper.
Legal and Administrative Requirements
Bringing a valid identification document (national ID card or birth certificate) is mandatory for hospital admission.
For patients under 18 years of age, the presence of a legal guardian is required.
Please ensure that one immediate family member accompanies you on the day of surgery.

Essential Post‑Rhinoplasty Care Guidelines
Strict Bed Rest? Not Recommended!
You are not required to remain in complete bed rest after rhinoplasty. Excessive sleep can actually increase facial swelling and lead to a longer recovery period.
Light movement helps your recovery — short walks beginning the day after surgery are highly recommended.
Cold and Warm Compresses — A Key Step
Cold compresses play a major role within the first 36 hours after surgery to reduce swelling and bruising. Use a cold gel ice pack or a special eye mask‑shaped compress available from pharmacies.
Apply the cold compress for five minutes every fifteen minutes to help minimize bruising and puffiness.
After completing the cold‑compress phase, starting from the second day, switch to warm compresses around the nose to speed up the healing process.
You can use a warm towel or a specially designed pad. Continue daily warm compresses for at least one week, or until swelling and bruising are gone.
Sleeping Position and Activity
For the first two weeks, keep your head elevated while sleeping — use two or three pillows.
Sleeping on your side is allowed and does not cause nasal deviation.
Avoid prolonged rest or sleep; it makes your face puffier. Begin short walks the day after surgery to promote circulation and faster recovery.
Nasal Dressing and Splints
Remove the under‑nose dressing one day after surgery.
If there are no secretions or bleeding, replacement of the dressing is unnecessary.
Inside the nose, a yellow mesh (splint) is usually placed during surgery and removed by the doctor 2–3 days later in-office.
Removal is quick and painless.
Do not confuse the mesh with a tampon. Tampons are no longer used in modern rhinoplasty, and Dr. Farshid Achak never uses nasal tampons for his patients.
In some cases, instead of mesh, silicone splints are placed and sutured to the inner mucosa. These should only be removed by the surgeon, typically two weeks after surgery.
Avoid any manipulation or touching of the nose for at least two months post‑surgery.
Using Medications After Rhinoplasty
Antibiotics: Start taking Cephalexin 500 mg capsules every 6 hours for 7–10 days, right after discharge, as directed by your doctor.
If you have any problems taking your medications, contact us immediately.
Nasal Sprays:
Two sprays are usually prescribed. Begin using them three days after surgery:
Debi‑Saline (or Rhinosaline) — a saline solution that cleanses the nasal passages. Continue using this spray for several months after surgery to prevent odor from internal sutures and help proper healing.
Rhinosoline or Oxymetazoline — use only for one week post‑operation, strictly according to medical instructions.
Prolonged use beyond one week is not permitted.
Bathing and Showering
Avoid washing your face during showers for the first week after surgery.
You may wash your body and hair only — but avoid showering for the first 48 hours after rhinoplasty.
Before your splint removal appointment at the clinic, it’s recommended to take a full shower, including gentle washing of the face.
Do not attempt to remove the splint (the “nasal cast”) yourself.
Exercising After Nose Surgery
Avoid intense exercises, bending, or activities requiring force for at least 8–10 weeks after surgery.
Daily light walking is strongly recommended — it improves circulation and reduces swelling faster.
Nutrition After Rhinoplasty
On the day after surgery, consume soft and light foods such as soup and fruit juice.
Starting from the second day, you can resume a regular diet, avoiding hard or crunchy foods like crusts or fried rice.
Pineapple and celery may be consumed optionally — but if you have acne or skin sensitivity, avoid pineapple; if you have digestive issues, avoid celery.
Minimize salt intake, as salt aggravates nasal swelling.
Remember: rhinoplasty is a light procedure, blood loss is minimal, and no special diet or supplements are required for recovery.

Frequently Asked Questions (FAQ)
Rhinoplasty primarily focuses on improving the appearance and aesthetic shape of the nose.
In contrast, Septoplasty is performed to correct deviation of the nasal septum and improve issues such as breathing difficulties, nasal bleeding, and pain.
When these two procedures are performed together, the surgery is referred to as Septorhinoplasty.
