Otoplasty (Cosmetic Ear Surgery)

Otoplasty, also known as cosmetic ear surgery, is performed on the visible part of the outer ear. It is a surgical procedure used to change the shape, position, or size of the ears.

The ear is composed of cartilage folds covered by skin. Ear development begins before birth and continues to progress during the years following birth.

Otoplasty can be performed at any age once the ears have reached their full size—typically from around 5 years of age through adulthood. At very young ages, the ear cartilage is too soft to hold sutures effectively.

If a child is born with prominent ears or other ear shape deformities, ear splinting may successfully correct these issues when started immediately after birth.

Types of Cosmetic Ear Surgery (Otoplasty)

Ear Pinning

This type of otoplasty involves bringing the ears closer to the head. It is performed on individuals whose ears protrude noticeably from the sides of the head.

Ear Reduction (Macrotia)

Macrotia refers to a condition in which the ears are larger than normal. Individuals with macrotia may undergo cosmetic ear surgery to reduce the size of their ears and achieve better facial balance.

Ear Augmentation

Some individuals may have small ears or ears that are underdeveloped. In such cases, cosmetic ear surgery may be performed to increase the size of the outer ear and improve its appearance.

Preoperative Preparation for Cosmetic Ear Surgery (Otoplasty)

You will discuss otoplasty with an ear, nose, and throat (ENT) surgeon. During your initial consultation, your surgeon will likely do the following:

Review Your Medical History

Be prepared to answer questions about your current and past medical conditions, especially if you have had any history of ear infections, which should be clearly reported to your doctor.

Your surgeon may also ask about medications you are currently taking or have recently taken, as well as any previous surgeries you have undergone.

Perform a Physical Examination

To determine the most appropriate treatment options, the doctor will examine your ears, including their position, size, shape, and symmetry.

The surgeon may also take photographs of your ears for your medical record.

Discuss Your Expectations

Your doctor will likely ask why you want to undergo otoplasty and what results you expect after surgery.

Make sure you fully understand the potential risks of otoplasty, including the possibility of overcorrection.

Preoperative Care Before Cosmetic Ear Surgery (Otoplasty)

  • Before undergoing cosmetic ear surgery, you should observe the following:

    • You may need to avoid aspirin, anti‑inflammatory medications, and herbal supplements, as these can increase the risk of bleeding.

    • Smoking reduces blood flow to the skin and can slow the healing process. If you smoke, your doctor will advise you to stop smoking before surgery and during the recovery period.

Postoperative Care After Cosmetic Ear Surgery

After cosmetic ear surgery, please follow these instructions:

  • Avoid touching or scratching your ears.

  • Choose a sleeping position that does not put pressure on the ears. To prevent pressure, avoid sleeping on your side.

  • Wear clothing that does not need to be pulled over the head, such as button‑down shirts.

Otoplasty Surgical Techniques

Otoplasty techniques vary depending on the type of correction required. The specific technique chosen by your plastic surgeon will determine the location of the incisions and the resulting scars.

The surgeon may place incisions behind the ears or within the inner folds of the ears.

Correction of Prominent Ears

Correction of prominent ears involves surgical techniques designed to create or enhance the folds along the rim of the ear and to reduce the size of the conchal cartilage (the largest and deepest depression of the external ear).

Incisions and Surgical Approach

Incisions for otoplasty are generally made on the posterior surface of the ear.

When incisions on the front of the ear are necessary, they are placed within the natural ear folds to keep them well concealed.

Internal sutures are used to secure the reshaped cartilage in its new position.

This procedure typically takes about two hours.

Anesthesia, Recovery, and Dressing

Otoplasty is not painful, as patients undergo the procedure under general anesthesia.

After surgery, with the ear positioned closer to the head, surgical scars are either hidden behind the ear or well concealed within the natural ear folds.

Your ears will be covered with bandages for protection and support. The dressing usually remains in place for about one week. Be sure to keep the dressing clean and dry.

You may experience mild discomfort and itching. Take pain‑relief medications exactly as prescribed by your doctor.
